      <abstract>
        <p>A review links maxillofacial trauma to psychological stress responses. It discusses screening, early interventions, and multidisciplinary care to improve recovery.</p>

      <title>Introduction:</title>

      <p>Maxillofacial trauma, the hard and/or soft trauma to the maxillofacial region, may involve fracture of dental tissue, the mandible <xref ref-type="bibr" rid="ridm1849716748">1</xref><xref ref-type="bibr" rid="ridm1849787084">2</xref><xref ref-type="bibr" rid="ridm1849816780">3</xref><xref ref-type="bibr" rid="ridm1849575180">4</xref> zygomatic  bone <xref ref-type="bibr" rid="ridm1849716748">1</xref><xref ref-type="bibr" rid="ridm1849787084">2</xref><xref ref-type="bibr" rid="ridm1849816780">3</xref><xref ref-type="bibr" rid="ridm1849575180">4</xref> maxilla <xref ref-type="bibr" rid="ridm1849716748">1</xref><xref ref-type="bibr" rid="ridm1849787084">2</xref><xref ref-type="bibr" rid="ridm1849816780">3</xref><xref ref-type="bibr" rid="ridm1849575180">4</xref> nasal <xref ref-type="bibr" rid="ridm1849716748">1</xref><xref ref-type="bibr" rid="ridm1849787084">2</xref> and frontal bone <xref ref-type="bibr" rid="ridm1849716748">1</xref><xref ref-type="bibr" rid="ridm1849787084">2</xref><xref ref-type="bibr" rid="ridm1849816780">3</xref> There may also be concomitant             abrasion <xref ref-type="bibr" rid="ridm1849578204">5</xref><xref ref-type="bibr" rid="ridm1849566412">6</xref><xref ref-type="bibr" rid="ridm1849563316">7</xref> laceration <xref ref-type="bibr" rid="ridm1849578204">5</xref><xref ref-type="bibr" rid="ridm1849566412">6</xref><xref ref-type="bibr" rid="ridm1849563316">7</xref><xref ref-type="bibr" rid="ridm1849551380">8</xref> or contusion of soft tissues <xref ref-type="bibr" rid="ridm1849578204">5</xref><xref ref-type="bibr" rid="ridm1849566412">6</xref><xref ref-type="bibr" rid="ridm1849563316">7</xref>. It is often a painful experience in body and soul, given that it could easily be a life and death experience from sports <xref ref-type="bibr" rid="ridm1849716748">1</xref><xref ref-type="bibr" rid="ridm1849555268">9</xref> falls <xref ref-type="bibr" rid="ridm1849716748">1</xref><xref ref-type="bibr" rid="ridm1849578204">5</xref><xref ref-type="bibr" rid="ridm1849555268">9</xref><xref ref-type="bibr" rid="ridm1849543740">10</xref> physical contact <xref ref-type="bibr" rid="ridm1849716748">1</xref> violence <xref ref-type="bibr" rid="ridm1849543740">10</xref> to road traffic accidents <xref ref-type="bibr" rid="ridm1849716748">1</xref><xref ref-type="bibr" rid="ridm1849578204">5</xref>. </p>
      <p>Treatment for maxillofacial trauma may vary from simple observation <xref ref-type="bibr" rid="ridm1849538268">11</xref> to splinting <xref ref-type="bibr" rid="ridm1849787084">2</xref><xref ref-type="bibr" rid="ridm1849555268">9</xref> wiring <xref ref-type="bibr" rid="ridm1849787084">2</xref><xref ref-type="bibr" rid="ridm1849555268">9</xref><xref ref-type="bibr" rid="ridm1849538268">11</xref> extractions <xref ref-type="bibr" rid="ridm1849555268">9</xref>, or open <xref ref-type="bibr" rid="ridm1849551380">8</xref><xref ref-type="bibr" rid="ridm1849555268">9</xref><xref ref-type="bibr" rid="ridm1849538268">11</xref> closed reduction <xref ref-type="bibr" rid="ridm1849551380">8</xref><xref ref-type="bibr" rid="ridm1849538268">11</xref> with/ without internal fixation with bone plates <xref ref-type="bibr" rid="ridm1849551380">8</xref><xref ref-type="bibr" rid="ridm1849555268">9</xref><xref ref-type="bibr" rid="ridm1849538268">11</xref>. Each stage of management from the initial presentation, treatment and   rehabilitation and recovery and follow-up may all post severe blows to the already traumatized patient. Such patients may be faced not only with aesthetic <xref ref-type="bibr" rid="ridm1849473788">20</xref> but also functional issues <xref ref-type="bibr" rid="ridm1849496988">16</xref><xref ref-type="bibr" rid="ridm1849473788">20</xref>. </p>
      <p>Single or multiple surgical interventions may be necessary. Eventual healing and rehabilitation could feel forlorn with uncertain outcome. On top of all these, other boggling issues include financial ability <xref ref-type="bibr" rid="ridm1849531356">13</xref> social aspect <xref ref-type="bibr" rid="ridm1849536468">12</xref> employment issues <xref ref-type="bibr" rid="ridm1849536468">12</xref><xref ref-type="bibr" rid="ridm1849531356">13</xref> and underlying medical issues <xref ref-type="bibr" rid="ridm1849531356">13</xref> etc. A lot of such trauma patients tend to be associated with increased social anxiety and avoidance <xref ref-type="bibr" rid="ridm1849504908">14</xref><xref ref-type="bibr" rid="ridm1849500372">15</xref> depression <xref ref-type="bibr" rid="ridm1849504908">14</xref><xref ref-type="bibr" rid="ridm1849500372">15</xref> low               self-concept <xref ref-type="bibr" rid="ridm1849536468">12</xref><xref ref-type="bibr" rid="ridm1849496988">16</xref> problems with relationships <xref ref-type="bibr" rid="ridm1849536468">12</xref> and difficulties withemployment <xref ref-type="bibr" rid="ridm1849500372">15</xref>. All these does appear to negatively impact on the quality of life of such patients <xref ref-type="bibr" rid="ridm1849496988">16</xref><xref ref-type="bibr" rid="ridm1849510020">17</xref><xref ref-type="bibr" rid="ridm1849482716">18</xref>.</p>
      <p>Often, cultural acceptance <xref ref-type="bibr" rid="ridm1849536468">12</xref> and social support <xref ref-type="bibr" rid="ridm1849510020">17</xref> plays an important role in the   psychological wellbeing of such patients <xref ref-type="bibr" rid="ridm1849536468">12</xref><xref ref-type="bibr" rid="ridm1849478108">19</xref>. Consequently, it is not difficult to imagine that patients suffering from maxillofacial trauma could have adjustment and adaptation issues stemming from both the trauma and treatment for the trauma <xref ref-type="bibr" rid="ridm1849531356">13</xref>. Some such patients may have to come to terms with change in appearance <xref ref-type="bibr" rid="ridm1849536468">12</xref><xref ref-type="bibr" rid="ridm1849482716">18</xref> aesthetics <xref ref-type="bibr" rid="ridm1849536468">12</xref> and functional issues <xref ref-type="bibr" rid="ridm1849531356">13</xref><xref ref-type="bibr" rid="ridm1849504908">14</xref><xref ref-type="bibr" rid="ridm1849496988">16</xref> that may correlate more to the subjective severity of either the surgical operation or outcome <xref ref-type="bibr" rid="ridm1849504908">14</xref><xref ref-type="bibr" rid="ridm1849500372">15</xref><xref ref-type="bibr" rid="ridm1849478108">19</xref><xref ref-type="bibr" rid="ridm1849473788">20</xref>. </p>
      <p>It had been reported that the degree of anxiety in patients was directly proportional to the magnitude of injury and the resulting scar <xref ref-type="bibr" rid="ridm1849482716">18</xref>. There has been technological advances to avoid scars <xref ref-type="bibr" rid="ridm1849787084">2</xref><xref ref-type="bibr" rid="ridm1849538268">11</xref>. On the other hand, it has been reported that approximately 20% - 40% of patients suffering from maxillofacial trauma may still develop post-traumatic stress disorder <xref ref-type="bibr" rid="ridm1849531356">13</xref><xref ref-type="bibr" rid="ridm1849504908">14</xref><xref ref-type="bibr" rid="ridm1849500372">15</xref><xref ref-type="bibr" rid="ridm1849473788">20</xref>. Islam et al’s study <xref ref-type="bibr" rid="ridm1849473788">20</xref> has shown a      nine-fold increase in the risk of depression (odds ratio of 9.02) and a two-fold increase in anxiety disorder (odds ratio or 2.68) in participants with facial trauma. Similar results were obtained in Gandjalikhan-Nassab et al’s 2016 study <xref ref-type="bibr" rid="ridm1849482716">18</xref>. </p>
      <p>One resulting concern for the healthcare team is that psychological stress of patients may potentially complicate recovery and adversely affect patient compliance <xref ref-type="bibr" rid="ridm1849473788">20</xref>. As such, assessment and provision of psycho-emotional support to patients suffering from maxillofacial trauma, an area less studied, has been gaining interest and importance and protocols put forward <xref ref-type="bibr" rid="ridm1849536468">12</xref><xref ref-type="bibr" rid="ridm1849531356">13</xref><xref ref-type="bibr" rid="ridm1849482716">18</xref><xref ref-type="bibr" rid="ridm1849478108">19</xref>.</p>
